Plastic, reconstructive and aesthetic surgery: a perspective Plastic, reconstructive and aesthetic surgery is an exciting, modern, innovative surgical specialty, with a long and distinguished history. Unlike other surgical disciplines, the field of plastic surgery is a specialty primarily based on techniques rather than pathologies or organ systems in isolated anatomical areas. This makes plastic surgery somewhat difficult to define, due to its complexity and the wide range of surgical techniques it employs. The art and science of plastic surgery remains the last true general surgical specialty—treating a wide range of patients, of all ages, covering all anatomical areas, as well as the whole spectrum of pathologies. The majority of plastic surgery work covers congenital, degenerative, traumatic, infective and neoplastic disorders. This diversity means that plastic surgeons interface with more medical, surgical and allied health specialties than any other field. Multidisciplinary working also ensures that multiple solutions are possible to treat and manage each patient’s health challenges, and allows for a precise analytical selection of procedures, constant improvement, as well as the development of innovative strategies. In the evolution of the management of complex wounds, plastic surgeons have a special role as the ‘leaders of last resort’, being the specialists for all types of complex wounds of different aetiologies—from burns, tissue injuries and radiation injuries to trauma wounds, or degenerative or neoplastic changes. Operative techniques and preferences may differ among plastic surgeons and, in many cases, there is no single right answer; however, the challenge for every surgeon is to think critically, use all of their skills and knowledge to diagnose each patient, and select the most appropriate surgical operation and technique, based on their personal experience and expertise, as well as the latest clinical evidence, in order to achieve the very best outcome for each patient. The articles in the Journal of Wound Care (JWC) aim for definition: of incidence; aetiology; general clinical presentation; classification; medical history; options; surgery; effect of treatment/surgery in particular clinical settings. Surgical techniques are presented by description, indication, principles, technique, complications, and outcomes, to improve understanding. Diseases are discussed in the context of the latest insights into the treatment and management of complex wounds in order to achieve the best possible outcome for our patients. JWC supports the ongoing learning and sharing of innovation, and best practice among the entire health community.
