A very little key to open a very heavy door Challenges in wound healing set the backdrop for our clinical lives. It may be a tad early for Christmas references, but I just cannot resist a reference to Dickens. Written in 1843, A Christmas Carol is set in a Victorian London with the contrast of prosperity and the deeply impoverished population. His main character Scrooge realises it is never too late to change. There can be a direct analogy to wound healing: the common theme of ‘past, present and future’. With small changes can come major advances. Small clues can lead to transformational solutions. In his own words ‘A hair or two will show where a lion is hidden. A very little key will open a very heavy door.’ Negin Shamsian MBChB MRCS MD Editor Journal of Wound Care In challenging wounds, our powers of observation and investigation of aetiology can often unlock the heavy door of healing for hard-to-heal wounds. In October’s edition of JWC we take a deeper look at challenging wounds and advanced solutions. Coltro and colleagues from Brazil illustrate treatment of complex palmar–plantar wounds using a bilaminar ‘trapdoor’ technique. Ricci and colleagues from Italy describe wound bed preparation with hypochlorous acid oxidising solution (AOS) and standard of care (SoC) in patients with hard-to-heal ulcers. Allen and colleagues from the US demonstrate the use of the palliative care aspects of wound healing in an older, frail patient with complex health needs. The characteristics of new patient referrals to specialised diabetic foot units across Europe and the factors influencing delays is the subject of a prospective observational study of 332 consecutive patients, and quality of life improvement in patients with hard-to-heal leg wounds treated with Prontosan wound irrigation solution and wound gel is also examined in a prospective, open-label, single-arm, five-centre study of non-hospitalised patients. A systematic review examines the impact of topical oxygen therapy on diabetic foot ulcer healing rates. JWC also presents the report of the first meeting of a new Diabetic Foot Ulcer Advisory Board, Eastern Europe, established to inspire and promote continuous improvement in treatment protocols and quality of care for patients with diabetic foot syndrome.
